This photo portraying a funny classroom situation with George W. Bush is actually just a manipulated associated press photo that has undergone digital editing.
The proof of this can be found upon close inspection of the picture on the back of the book. You can see that the book President Bush is holding and the one the child is holding are mirror images of each other, clearly indicating that the photo was manipulated:
